Consent

This form should be used by the parents of children applying for EMSFR (need to be 16 yrs. old) or EMT-B licensure (need to be 17 yrs. old). Anyone 18 yrs. old does not need this form.

Background Screening

The EMSB has joined the majority of states in the country by requiring all applicants for licensure, and current license holders, to submit to a state and nationwide criminal background check. Please see the following publications for more information.
